A completed application packet includes the following:

Cover letter A completed Civil Service Form SD 5
Resume Cover letter
Three current letters of recommendation Resume (recommended)
College Transcripts or College Placement File Certification and/or License
Certification High School Diploma

Electronic copies of the documents listed above can be attached to the application on- line. If you are unable to submit electronic documents, you may submit paper copies by mail to the address above to the attention of the Human Resources Department.
